The MCHC TeleHealth Program offers video conference via Zoom meeting for HealthCare. The Zoom platform allows you to join in a private conference with your provider from your phone, tablet, computer, or even a standard phone.

Telehealth is open:

Monday - Friday: 8:30 AM - 8:00 PM

Saturday - Sunday: 8:30 AM - 5:00 PM

For new patients to MCHC please visit our Patient Registration page. When prompted fill out and submit the packet to be added to our medical records. For more information or assistance please call your local MCHC or the Telehealth numbers provided.

For current patients, please see our Telehealth Consent Form. When prompted fill out the consent form in order to be seen via Telehealth with your provider.

IMPORTANT Telehealth Information

To have a Telehealth visit with your provider over your handheld device, download the Zoom App. Click on the links for you corresponding device.

To Join from an iPhone, iPad, or Android Device

  1.  Open the app.

  2. Tap "Join a meeting".  (If this is the first Zoom meeting that you've joined, you will be asked to give access to your microphone).

  3. Enter the Meeting ID provided to you.

  4. When prompted tap "Wifi or Cellular Data", "Dial in" or "Call my phone".

NOTE: A Meeting ID and password will be provided by a Telehealth Facilitator in order for you to join. You will receive this information either over the phone, through text or email from the TeleHealth Facilitator and your provider.

To Join from a Mac, PC, or Windows

  1. Open Zoom in your web browser.

  2. Click Join at the top right of the page. Enter the Meeting ID or Personal Link Name.

  3. If the Zoom software isn't installed already the download will automatically start.

  4. After the download is finished install the software.

  5. Zoom will start and your meeting with your provider will start.

  6. It will ask how you want to join the audio. Choose the setting that best fits you.



NOTE: Please be aware that to have a Telehealth visit over these devices, you must have a type of microphone (internal or external) either with headphones or speakers. A webcam is not required.
