RULES OF CONDUCT AND RESPONSIBILITY FOR THE

 

EMPLOYEE WELLNESS CENTER (EWC)

 

NOTICE:

 

THE EWC IS AN UNSUPERVISED FACILITY. THE EMPLOYEE, THEIR GUEST,

IF ANY, OR OTHER PERSONS ASSUMES ALL RISK OF INJURIES AND LOSS

FOR ANY REASON THAT MIGHT OCCUR AS A RESULT OF THEIR USE OF, OR

PRESENCE IN, THE EWC, THE COMMON AREAS AROUND IT, AND/OR THE

MCHC CAMPUS IN/ON WHICH THE EWC IS LOCATED.

 

IF YOU DO NOT AGREE WITH THE ASSUMPTION OF RISK AS SET OUT IN THE

ABOVE PARAGRAPH, OR THE OTHER RULES AS STATED HEREAFTER, THEN

YOU ARE NOT PERMITTED TO USE OR BE PRESENT IN THE EWC.

 

DAYS/HOURS OF OPERATION:

 

- OPEN SEVEN DAYS A WEEK

- OPEN FROM 6 A.M. TO 10 P.M. EACH DAY

 

IN GENERAL:

 

- RESTRICTED TO THE USE OF MCHC EMPLOYEES, THEIR GUEST, AND

MCHC BOARD OF DIRECTORS.

 

- NO PERSON SHOULD USE THE EWC UNTIL THEIR PERSONAL HEALTH

CARE PROFESSIONAL HAS APPROVED THEIR ABILITY TO PARTICIPATE

IN AN EXERCISE PROGRAM.

 

- EMPLOYEES MUST ATTEND REQUIRED ORIENTATION WITH APPROVED

TRAINER FOR SAFE EQUIPMENT USE BEFORE GIVEN ACCESS TO EWC.

 

- EMPLOYEES MAY USE THE EWC DURING THE POSTED HOURS OF

OPERATION ONLY.

 

- APPRORIATE ATTIRE REQUIRED (ATHLETIC CLOTHING ONLY; NO CUT

OFFS, DRESS SHOES, SANDALS, ETC.)

 

- EMPLOYEES ARE ALLOWED ONE GUEST TO ACCOMPANY THEM WHILE

USING THE EWC.

 

- EMPLOYEES MUST CLEAN THE MACHINES THAT THEY AND THEIR

GUEST USE AFTER EACH USE. APPROPRIATE CLEANERS FOR

UPHOLSTERY AND NON-POROUS SURFACES ARE CLEARLY MARKED AT

STATIONS NEAR DOORS.

 

- EMPLOYEES AND GUESTS ARE TO BE COURTEOUS TO OTHER USERS.

 

- NO EMPLOYEE OR GUEST SHALL INDULGE IN HORSEPLAY, LOUD

AND/OR BOISTEROUS BEHAVIOR OR OTHER WISE CAUSE A

DISTURBANCE, DISTRACTION OR IN ANYWAY HARASS, EMBARRASS OR

INTIMIDATE OTHER EMPLOYEES OR THEIR GUESTS.

 

- NO EMPLOYEE OR GUEST SHALL MONOPOLIZE ANY PARTICULAR PIECE

OF EQUIPMENT WHEN SOMEONE ELSE IS WAITING.

 

- NO OUTSIDE FITNESS EQUIPMENT MAY BE BROUGHT INTO EWC.

 

- NO FOOD OR DRINK ALLOWED EXCEPT WATER AND SPORTS

THEMED BEVERAGE SUCH AS GATORADE.

 

CHILDREN:

- EMPLOYEES MAY NOT BRING CHILDREN AS GUESTS UNLESS THE

EMPLOYEE IS THE PARENT, LEGAL GUARDIAN, HAVE A VALID POWER

OF ATTORNEY OR HAVE LEGAL RESPONSIBILITY FOR SAID CHILD.

 

- ALL CHILDREN MUST VACATE THE EWC BY 7 PM EACH DAY.

 

- CHILDREN UNDER SIX YEARS OF AGE ARE NOT ALLOWED IN THE EWC.

 

- CHILDREN FROM SIX YEARS OF AGE THROUGH 12 YEARS OF AGE MAY

ACCOMPANY THE SPONSORING EMPLOYEE BUT ARE NOT ALLOWED TO

USE THE EXERCISE EQUIPMENT.

 

- CHILDREN FROM 13 YEARS OF AGE TO 18 YEARS OF AGE MAY USE THE

EQUIPMENT BUT MUST BE SUPERVISED AND ACTIVELY ASSISTED BY

THE SPONSORING EMPLOYEE WHILE USING EQUIPMENT.

 

GUESTS:

- NO MORE THAN ONE (1) GUEST AT A TIME PER SPONSORING EMPLOYEE.

 

- SPONSORING EMPLOYEE MUST ACCOMPANY GUEST AT ALL TIMES.

 

- SPONSORING EMPLOYEE IS RESPONSIBLE FOR GUEST CONDUCT,

INCLUDING ANY DAMAGES, INJURIES, VANDALISM, OR THEFT.
