Meet the Team!

Chasity Eversole

RN, CDCES,

MLDE, CPT

Chasity Eversole is a registered nurse (RN), Certified Diabetes Care and Education Specialist, Master Licensed Diabetes Educator (MLDE), and KY native.

Chasity graduated from Hazard Community and Technical College with an Associate Degree in nursing. She received her license through the Kentucky Board of Licensed Diabetes Educators and went on to become certified as a Diabetes Care and Education Specialist through the Certification Board for Diabetes Care and Education.

She enjoys working with patient to assist them through the challenges of managing diabetes.

Erin Gibson

MS, RD, LD, LDE

Erin Gibson, is a Registered Dietitian (RD), Licensed Dietitian (LD) and Licensed Diabetes Educator (LDE). She is a Hazard, KY native.

She graduated from the University of Kentucky with a Bachelor of Science in Dietetics and also completed her Master of Science in Community Nutrition and Dietetic Internship through Eastern Kentucky University. She is licensed through Kentucky Board of Licensed Diabetes Educators.

Mary Collins

APRN, DNP,

BC-ADM,

CDE, MLDE

Dr. Mary Collins is an Advanced Practice Registered Nurse (APRN). Additionally, she has earned her Doctorate in Nursing Practice (DNP) through Chamberlain University and board certification in Advanced Diabetes Management (BC-ADM).

 

She is a Certified Diabetes Education Specialist (CDE) and holds a Master's License in Diabetes Education (MLDE) through the Kentucky Board of Diabetes Educators. She has also been an insulin pump trainer for various brands. Mary is a Letcher County resident, and her lifelong desire is to help people with diabetes achieve their highest level of health and quality of life!

Mendy Boggs

MA, CCHW

Melissa McKnight, CNA, and Mendy Boggs, MA, CCHW, are part of MCHC's Diabetes Community Health Working program assisting patients with diabetes to live healthier lives.

 

The CHWs visit patient's homes weekly working directly with patients and their family to assist with:

Goal Setting based on the patients needs

Support to reach those goals

Explaining new and old prescriptions

Encouragement to make and keep health care appointments

Connecting patients to community resources

Nutrition and physical activity encouragement

The patient's role is to:

Check and keep a record of blood sugars

Stick to a medication plan

Be available to meet with CHW

Keep the CHW and provider informed of any ER visits or hospitalizations

Ask questions
